Update on 29.01.21: Notice of Closure
Following a consultation with patients the closure of the Clover practice branch surgery at Jordanthorpe Health Centre has now been approved by the Clinical Commissioning Group. You can read a copy of the patient consultation outcome report here.
We are now working towards a closure date of Sunday 28th February 2021 with the last day seeing patients Friday 26th February 2021.
Patients who do not wish to access their future care at Clover Practice’s other sites at Darnall or Tinsley are required to re-register with another surgery which covers your postcode. All local practices are currently accepting new patients. We are currently working closely with all our patients with complex needs to ensure they are supported to transition to a new practice.
Update on 21.01.21
The patient consultation feedback is being reviewed and we expect to be able to publicise the decision on whether or not the practice will close at the end of February in the coming week.
IMPORTANT LETTER TO CLOVER PRACTICE PATIENTS REGISTERED AT JORDATHORPE HEALTH CENTRE BRANCH SURGERY
Monday 12th October 2020
Clover Practice Jordanthorpe Health Centre Patient Consultation
Clover Practice has made the difficult decision to commence a consultation with our patients and stakeholders to propose closure of our Jordanthorpe Health Centre branch surgery. The decision to request the closure of the branch surgery has not been taken lightly and the wellbeing of patients remains the priority.
Why is the change being proposed?
Clover Practice currently runs four practices across Sheffield. The practice has advised NHS Sheffield Clinical Commissioning Group (CCG) that it is no longer sustainable to continue to run the branch surgery based at Jordanthorpe Health Centre. There are a number of other practices in the area, including one based in the Health Centre, which patients will be able to register with. We believe that patients of Clover Practice moving to other local practices will allow more NHS funding coming into the area to be spent on direct patient care, ensuring that you are cared for in the best possible way.
We have a tremendous practice team who have worked very hard to deliver the best possible care to patients despite the challenges, however the most recent GP survey shows that 58% of Clover practice patients describe their overall experience of the service as ‘good’ compared to 66% and 87% of patients at the two closest surgeries. We therefore believe that if the proposal for the branch surgery to close is approved, the health and wellbeing of patients will not be affected.
What this may mean for patients
We are proposing the closure of the branch surgery based in Jordanthorpe Health Centre. If the proposal is accepted, you will be required to re-register with another surgery which covers your postcode. All local practices are currently accepting new patients.
If the proposal is accepted, we will make sure that patients with complex needs are supported to transition to a new practice by working with their new GP.
How you can have your say
A formal consultation about the proposed closure of the Clover Practice Jordanthorpe Branch Surgery will begin on Monday 12th October 2020. The consultation will take place over 6 weeks, ending on Sunday 22nd November.
The purpose of the consultation is to understand and consider the views of our patients and stakeholders on the proposal, and understand more fully what the impacts of the change may be.
We welcome all of your views and you can take part in the consultation by completing the patient questionnaire. The questionnaire is also available online at http://bit.ly/JordanthorpeConsultation
Please contact us on 0114 237 1160 if you require additional printed copies of the questionnaire, need the information in another format or language, or need help completing the survey. Copies of the questionnaire will also be available from the reception desk at the surgery.
We will be holding an online patient consultation meeting to answer any questions you may have. This meeting will be held on Tuesday 3rd November at 2pm. There will be a senior member of the Practice team available at the session to answer your questions. Register in advance for this meeting here: http://bit.ly/JordanthorpeMeeting. After registering, you will receive a confirmation email containing information about joining the meeting.
Informing you of the outcome of the consultation
Once the consultation closes, an outcome report will be written which will include a full analysis of the responses, key issues and recommendations. The outcome report will then be presented to NHS Sheffield CCG for consideration. Following consideration and a decision being made, the outcome will be shared with patients in the same way as you have received this letter. Should the proposal be accepted then all patients will be contacted advising them on how to register with a new practice.
We understand that some patients will be disappointed to hear of this proposal, we welcome you to share your views in our consultation and ask for your understanding and support to work with us through this proposed change.
Some of the initial patient feedback has suggested patients want a list of the practices they can register with locally. This does not predicate any decision on the closure, but responds to patients wanting more detail to understand their options.
